Parent/Guardian authorization for Georgia Cyber Academy to request student records from another educational institution

Parents and/or Guardians may authorize Georgia Cyber Academy to release student records to another school or academic program by completing and submitting the Parent Release of Student Records Authorization Form. Please submit form by email or fax:
