Easy to access

Sign in to Hazel Health by app or web, and request a medical visit.

Any enrolled student can access Hazel’s virtual healthcare services Just like a regular healthcare provider, Hazel bills the student’s health insurance for services rendered. Any remaining costs to the family (e.g., copays) are covered by our partnership with Hazel Health. If a student is verified as uninsured, their care is still 100% covered.

What parents can expect

Hazel verifies insurance status and collects insurance info during the enrollment process. Families with insurance can provide info in two ways:

1-Provide insurance info via the Hazel consent form

To receive Hazel’s services, a student’s guardian must complete a consent form. In the online consent form, families can upload photos of the front and back of their insurance card, or type in their insurance details manually.

2- Call Hazel Health directly

Families can submit their insurance information over the phone by calling Hazel Health directly at 1-888-541-7063.

Note: In some cases, during the call, the Hazel Health team will send a URL via text message to the family to submit insurance.

If a student is uninsured, they can still access Hazel's services at no cost. See hazel.co/help/faq for more info.

Hazel Health pediatric doctors, nurse practitioners, and physician assistants can help with respiratory issues (wheezing, coughing, asthma flares), eye irritation, rashes/skin concerns,
and medication refills.

New to our medical services? Read the steps below to learn how to start a physical health
visit for your child. Physical health visits are virtual, and can be accessed on any
smartphone, tablet, or computer connected to WiFi.

Step 1

Visit my.hazel.com/patient and click the blue Sign Up Now button if you do not already have an account.

To create your account, enter a phone number that can receive text messages and select Create
Account. If you already have an account, log in and skip to step 6.

Step 2

After creating an account, you will be prompted to add your child. You will need to enter their district
name, student ID, and their date of birth. Once you enter this information, select Next.

Step 3

If you have not yet completed consent, do so during this step. If your child has insurance, please be sure to fill this information out.

Step 4

Once your child has been added and consent has been completed, you will see this confirmation window. Select Next to return to the homepage.

Step 6

Once you select the child you are seeking care for, select Next.

Step 7

Enter the reason for your child’s visit and how you would like to be contacted, then select Send Message.

Step 8

After submitting the visit request, keep an eye out for a text message or phone call from our support team.

Step 9

Through the Hazel chat, a Hazel support team member will ask for more details on your child’s health concerns and reasons for requesting a visit.

Step 10

At the time of the visit, you will receive a text message with a link to join the visit.

Step 11

The link from the text message will navigate you to your Hazel account. You will see your child’s
appointment under “Active Visits,” click on the active visit to join the call.
