Proof of Enrollment Letter
Email enrollment@georgiacyber.org to initiate a request for a Proof of Enrollment Letter (for security reasons, please send request from email address on file).
Request MUST include the following:
- Your Child's Name
- Student ID Number
- Details About What You Are Requesting

Jury Duty Letter
Email records@georgiacyber.org to initiate a request for a Jury Duty Letter (for security reasons, please send request from email address on file).

Address Verification Letter
Email records@georgiacyber.org to initiate a request for an Address Verification Letter (for security reasons, please send request from email address on file).

Social Security Form
Email records@georgiacyber.org to initiate a request for a Social Security Form (for security reasons, please send request from email address on file).
Request MUST include the following:
- Your Child's Name
- Student ID Number
- Details About What You Are Requesting
Social Security forms will be placed in the mail within 5–7 school days.
IMPORTANT: Please allow 5–7 school days for processing of all requests.
Georgia Driver's License or Learner's Permit
You must be at least 15 years of age to apply for a learner's permit and you must be at least 16 years of age to apply for a provisional driver's license.

Certificate of Enrollment
Certificate of School Enrollment Form
- Download and Complete Part A Only:
GADDS Certificate of School Enrollment Form
- Fax (404.684.8830) or scan and email the form to enrollment@georgiacyber.org.
PLEASE NOTE: DDS only accepts original documents and will not accept photocopies of the notarized certificate of enrollment form.
Once you have submitted your Certificate of School Enrollment form please allow 5–7 school days for processing.
The Certificate of School Enrollment is good for 30 days after the date it is notarized.
Recently Withdrawn Students: During the student's SUMMER VACATION, DDS extends the expiration dates. Please contact your local DDS for the extended dates. (If you are a student who completed the most recent academic school year with GCA and withdrew after the last day of school or during the summer months, you may submit your Certificate of School Enrollment to be processed. Please submit your Certificate of School Enrollment before August 1.)
Recently Enrolled Students: Per DDS, the student must be currently enrolled with GCA to receive a Certificate of School Enrollment form. (All new students must be actively enrolled at GCA for 10 consecutive school days after the school's start date to obtain a Certificate of School Enrollment)
Once the form has been processed, a copy will be mailed to the address on the Certificate of School Enrollment form.

ADAP
Alcohol and Drug Awareness Program (ADAP)
By law, Georgia teens under the age of 18 are required to successfully complete the Alcohol and Drug Awareness Program in order to obtain their Class D Georgia driver’s license. GCA students can complete the eADAP Program online.

Community Service
Work Permit
Complete a work permit form found on the Georgia Department of Labor website: https://www.dol.state.ga.us/WS4-MW5/cics.jsp?TRANSID=WP17&FRMNAME=WP17
Fax (404.684.8830) or scan and email your work permit form to records@georgiacyber.org The work permit form must be signed by your employer.
Once you have submitted your work permit data sheet, the process may take 5–7 school days. You will receive a work permit certificate via email to take to your employer.
Please note that the form must be completely filled out by both you and your employer. This means you MUST have a job prior to requesting a work permit.
Records Request
To request student records:
A new school or academic program may submit a formal request by fax or email.
Parent/Guardian may request student records by submitting a Student Records Release Form by fax or email. Please be sure to include photo identification.

A high school transcript is a record of your academic accomplishments in high school. Transcripts are updated as courses are completed at the end of each semester.
