Proof of Enrollment Letter
Email enrollment@georgiacyber.org to initiate a request for a Proof of Enrollment Letter (for security reasons, please send request from email address on file).
Request MUST include the following:
- Your Child's Name
- Student ID Number
- Details About What You Are Requesting

Jury Duty Letter

Address Verification Letter

Social Security Form
Email records@georgiacyber.org to initiate a request for a Social Security Form (for security reasons, please send request from email address on file).
Request MUST include the following:
- Your Child's Name
- Student ID Number
- Details About What You Are Requesting
Social Security forms will be placed in the mail within 5–7 school days.
IMPORTANT: Please allow 5–7 school days for processing of all requests.
Georgia Driver's License or Learner's Permit
You must be at least 15 years of age to apply for a learner's permit and you must be at least 16 years of age to apply for a provisional driver's license.

Certificate of Enrollment
Certificate of School Enrollment Form
- Download and Complete Part A Only:
GADDS Certificate of School Enrollment Form
- Fax (770-372-5108) or scan and email the form to enrollment@georgiacyber.org.
Once you have submitted your Certificate of School Enrollment form please allow 5–7 school days for processing.
The Certificate of School Enrollment is good for 30 days after the date it is notarized.
Recently Withdrawn Students: During the student's SUMMER VACATION, DDS extends the expiration dates. Please contact your local DDS for the extended dates. (If you are a student who completed the most recent academic school year with GCA and withdrew after the last day of school or during the summer months, you may submit your Certificate of School Enrollment to be processed. Please submit your Certificate of School Enrollment before August 1.)
Recently Enrolled Students: Per DDS, the student must be currently enrolled with GCA to receive a Certificate of School Enrollment form. (All new students must be actively enrolled at GCA for 10 consecutive school days after the school's start date to obtain a Certificate of School Enrollment)
Once the form has been processed, a copy of the form will be emailed to the email address on file.

ADAP
Alcohol and Drug Awareness Program (ADAP)
By law, Georgia teens under the age of 18 are required to successfully complete the Alcohol and Drug Awareness Program in order to obtain their Class D Georgia driver’s license. GCA students can complete the eADAP Program online.

Community Service
Work Permit
Complete a work permit form found on the Georgia Department of Labor website: https://www.dol.state.ga.us/WS4-MW5/cics.jsp?TRANSID=WP17&FRMNAME=WP17
Fax (404.684.8830) or scan and email your work permit form to records@georgiacyber.org The work permit form must be signed by your employer.
Once you have submitted your work permit data sheet, the process may take 5–7 school days. You will receive a work permit certificate via email to take to your employer.
Please note that the form must be completely filled out by both you and your employer. This means you MUST have a job prior to requesting a work permit.

Academic Records Requests
Georgia Cyber Academy is now offering a secure website for student academic records requests.
Student Records RequesT Website
Per District policy and the Family Educational Rights and Privacy Act (FERPA) student records are only available to those who have authorization from the student or parent, guardian, or responsible person if the student is less than 18 years of age. If the student is 18 years of age or older, school records will not be released to a parent, guardian, or responsible person.
Charges will apply when ordering former student records.

Transcript Requests
Please note the information below:
- Students in Grades 09 - 11 must request transcripts through Parchment.com.
- Students in Grade 12 must request their official transcript through Naviance.
- Charges will apply when ordering former student records.
Fulfillment Time
Please allow 2-3 business days for processing record requests.
During periods of high volume (open enrollment, etc.), allow 7 to 10 business days for processing student record requests.
For all records requested through our Student Records Request website, emails notifying you of the status of your records request order will be sent from scribOnline@scribsoft.com. It is important you read those emails carefully as additional information may be required to process your request.
Processing time will be two business days from the time the order is received.

Georgia Milestones Scores
Georgia Milestones ScoresGeorgia Milestones End of Grade (EOG) test scores for grades 3–8 and End of Course (EOC) test scores for high school are available in the Infinite Campus parent portal. Please view the following document for instructions on accessing your student's EOG or EOC scores: Retrieving State Test Scores from the Infinite Campus Parent Portal.
If your student is withdrawing from GCA, a records request may be completed by the Legal Guardian and test records will be sent to the receiving school. If you would like to request records, please have the new school fax a records request or the Legal Guardian must complete the K–12 Student Records Release Form.
If your student has withdrawn and is not currently enrolled with GCA, you will need to submit a records request to receive a paper copy of your student's scores.

Under the Georgia Open Records Act (OCGA §50-18-70 to §50-18-77) all public records are open for personal inspection and copying, except those that are exempted from disclosure by order of a state of Georgia court or by law.

GCA will provide information relating to the request, as permissible and applicable by law, within 3 business days after it has been received. If production of the information is not possible within that timeframe, the GCA Operations Manager will issue correspondence with an explanation.

A high school transcript is a record of your academic accomplishments in high school. Transcripts are updated as courses are completed at the end of each semester.
