Completing the Online Application
New Applicants
After confirming your seat offer, you will receive a link to the Online Enrollment Application via email. Please have all required enrollment documentation available and set aside at least one hour of your time to complete the online application.
Follow the steps outlined below to complete the Online Enrollment Application.
New Student(s) Enrollment Application Steps
Before you begin, it is best to have documentation available in a digital format (photo, screenshot, or scan) so you can upload easily upon request. The following documents will be requested during the application process:
- Proof of Residence
- Parent/Legal Guardian Photo Identification
- Student Birth Certificate/Proof of Age
- Student Social Security Card or Waiver
- IEP/Special Education Documents (if applicable)
- 504 Plan Documents (if applicable)
- Gifted Eligibility Documents (if applicable)
- Report Card or High School Transcript
- GA Immunization Form 3231 or a Notarized Affidavit of Religious Objection to Immunization.
- GA Form 3300-Certificate of Vision, Hearing, Dental, and Nutrition Screening
- GA Form 4400 Scoliosis Screening (6th and 8th grade only)

- Select your preferred language (English or Spanish)

Enter the requested information to begin the registration process:

Select the box to indicate if your student previously attended GCA

Type the letters indicated in the image on screen (will differ from what is displayed in the screen capture below)

Select "Begin Registration"
- Check your email inbox or junk box for a message from donotreply@georgiacyber.org with subject line “Invitation to Complete GCA Enrollment Application” and select the link included in the email to begin the application.

Step 2: Starting the Enrollment Application
- Select your preferred language (English or Spanish). Write down or take a screen capture of your personal Application Number listed on screen. You may need to refer to this number in the future. (number will be different than what is displayed on the screen capture below)

Complete the following:

Type in your first and last name to attest that you are the person authenticated into this application or an authorized user of this account, and the data you are entering/verifying is accurate and true to the best of your knowledge.

Sign your name.

Select “Submit”

Read the information shown on screen and then select “Click here to get started”

Step 3: Add Student(s) Primary Household Information

Primary Phone - Enter the Primary Phone number then select “Next” to continue

Home Address - Complete the following:

Enter your home address into the fields listed on screen.

Verify that your address was entered in correctly.

Select “Upload Proof of Residence” to upload proof of Georgia residency documentation.

Select “Next” to continue.

Mailing Address - If your Mailing Address is the same as your Home Address, select the box next to “This household has no separate Mailing Address”. If your Mailing Address is different than your Home Address, uncheck the box and enter your Mailing Address. Select “Next” to continue.

Migrant Worker - Answer the questions on screen by selecting “Yes” or “No”. Select the appropriate boxes as necessary. Select “Next” to continue.
- Referral - Let us know how you found out about Georgia Cyber Academy (GCA). Select your choice from the drop-down menu. If not listed, select “other” and indicate how you found out about GCA. Select “Next” to continue.

Expectations - Complete the following:

Select the “please watch this short video” link to learn about the expectations of schooling at Georgia Cyber Academy.

Select the box to indicate you watched the video.

Select “Save/Continue”.

Select “Upload Legal Guardian Photo ID” to upload a copy of the Legal Guardian’s Photo Identification.

Contact Information - Complete the following:

Military Connections - Select “Yes” or “No” to the question on screen. Select “Save/Continue”.

- Add an additional Parent/Guardian
Once these actions are completed, select “Save/Continue”

Step 5: Add Emergency Contact Information
- Select “Add Emergency Contact” to add up to three Emergency Contacts who were not previously listed as Parent/Guardian contacts.

Step 6: Add Other Household Member Information
Complete this section to add other members who live in your household including students who do not attend GCA and any other adults who live in the home. Note: Parent(s)/Guardian(s) and enrolling student(s) do not need to be added here since they are listed elsewhere in the application.
Complete the "If Yes" instructions below if you do have other household members to add. Complete the "If No" instructions of you do not have other household members to add.
If Yes:
- Select “Add Another Household Member” to add any other members who live in your household including students who do not attend GCA and any other adults who live in the home.

Enter the Application ID number you received via email during the Lotterease process. (Note: The email sender will be "Georgia_Cyber_Academy@lotterease.com" with Subject "Enrollment Confirmed at Georgia Cyber Academy.)

Select the drop down arrow next to “Enrollment Plans” to continue.

Enrollment Plans – Select “Registering” to add a New Student Registration to the Enrollment Application. Select “Next” to continue.

Select “Yes” or “No” to indicate your option to add your student’s Social Security Number (SSN). (Note: Failure to provide student's SSN will inhibit Georgia HOPE scholarship eligibility.)

Select “Yes” or “No” to indicate whether your student is interested in college athletics

Select “Yes” to opt your student in to text messages or “No” to opt your student out of text messages.

Select “Upload Birth Certificate” to provide a copy of your student’s Birth Certificate .

Select “Upload Social Security Card” to provide a copy of your student’s Social Security Card or provide the Social Security Waiver.

Race/Ethnicity – Follow the on-screen instructions to complete the student’s race/ethnicity information. Select “Next” to continue.
- Language Information - follow the on-screen prompts to indicate your student's language background. Select "Next" to continue.
- Housing - Select from from the following options to indicate family's housing situation:
- Student Services - Choose "Yes" or "No" to each question regarding services your student receives or has received (Special Education, 504 Plan, Gifted Education). - Previous School - Follow the on-screen instructions to enter in information about the student's previous school. Upload transcript, report card, or progress report.

(Note: If your student has a a gap in their education, upload the GCA Statement Acknowledging Missing Attendance form.) Select "Next" to continue.
- All GCA students are required to have a Learning Coach. The Learning Coach may be the parent/guardian or another adult designated by the parent/guardian. Read the primary duties of the Learning Coach listed on screen then enter in the Learning Coach information as requested. Select "Next" to continue.
- Relationships-Parent/Guardians - Read the information on screen and follow the prompts to complete information about the Parent/Guardian. If necessary, upload Legal Guardianship Documentation or a Non-Parental Affidavit as outlined.

- Relationships-Emergency Contacts - Select the Emergency Contact(s) relationship and contact sequence. Select "Next" to continue.
- Other Household Members - Select the relationship of the other household members to the student. Select "Next" to continue.
- Health Services-Medical or Mental Health Conditions - Follow the on-screen prompts to indicate any medical or mental health conditions for the student.
- Health Services-Medications - Complete the following:
- Indicate any medications your student takes by selecting "Add Medication" or "No medications" and following any on-screen prompts.
- Upload a copy of the student's GA Immunization Form 3231 or a Notarized* Affidavit of Religious Objection to Immunization.
- Upload a copy of the student's GA Form 3300-Certificate of Vision, Hearing, Dental, and Nutrition Screening.

Select "Next" to continue.
- Equipment- Answer "Yes" or "No" to the following:
- Does your student currently have a GCA loaner computer?
- Does your student need a GCA loaner computer?
- Do you have daily access to the internet?
Select "Next" to continue.
- Release Agreements - Complete the following:
- Indicate whether you agree to the GCA Photo & Testimonial Release for this student by selecting "Yes" or "No".
- Indicate whether you agree to the GCA Liability Release for this student by selecting "Yes" or "No".
- Select the box to acknowledge that you understand the loaner computers are property of Georgia Cyber Academy and that once your student is no longer enrolled with GCA, you must return the loaner computer or will be charged for the stolen property.
- Select the box to acknowledge that you agree to the terms and conditions of the Technology Acceptable Use Policy.
- Sign in the box to confirm you understand the above statements and acknowledgements.
- Select "Save/Continue"
- The following options are available:
- Edit/Review student information
- Add another student.
Once complete, select "Save/Continue".
Move to Step 8: Complete the Application

Step 8: Complete the Application
- Complete the following:
- STOP! Read the message on screen to confirm that you have added all students who will be enrolling at Georgia Cyber Academy for the 2020-2021 school year to the Enrollment Application. If not, select "Back" to add another student(s).
- Download a PDF summary of the Enrollment Application
- Share the Good News about your student's Enrollment on Facebook.
- Select the "Submit" button to complete the Enrollment Application.
- You will receive a confirmation email from "donotreply@georgiacyber.org" with subject "GCA Enrollment Application - Status Update". Save this email as your confirmation. Make note of your Application Number listed in the email message.
- Your application will be reviewed by the Enrollment & Records Department. Should they require more information or clarification on information received, they will be in contact by email or phone.
