Required Enrollment Documents

After confirming your lottery selection, you will receive a link to complete the Online Enrollment Application.  All required documentation must be provided for our Enrollment & Records Department to successfully process your application. 

Required for enrollment:

  • Proof of Georgia Residency
  • Proof of Age
  • Social Security Card
  • Report Card or High School Transcript
  • Photo Identification

It is best to have all documents available in a digital format (photo, screenshot, or scan) so you can upload easily upon request.

* Health Forms

* The Georgia Department of Health requires these forms to be maintained on file for every student enrolled:

These health forms should be submitted while completing the application.  If a health form is not available at the time the application is completed, it can be sent by email to after enrollment is approved. 

Students will not be permitted to re-register or enroll for the next school year if any Georgia Department of Health form remain(s) outstanding from the student's file.
